By clicking on the ‘Join Now’ button on the online application form to join the Crescent Wealth Superannuation Fund, I:

• apply to be a member of the Crescent Wealth Superannuation Fund, and the application is made in Australia.
  
  
• acknowledge that I have received all the information I require to exercise the choices I have made.
  
  
• accept that I will be bound by the provisions of the trust deed and designated rules which govern the operation of the Superfund.
  
  
• understand and consent to my information being collected, disclosed, and used in the manner set out in the Privacy Policy.
  
  
• acknowledge that by providing my email address in the online application form, the trustee and fund may, at its discretion, use that email address:
  
  
  • to send me information concerning my super, member, and exit statements, notices or occurrence of significant events, and other member communications; and
    
    
• • for marketing and research purposes, including sending me information about other financial products or services offered by the trustee or fund.
    
    
• acknowledge that the trustee may provide any member communications (including annual reports, member and exit statements, notices or occurrence of significant events, and other member communications) and/or marketing and research material that are permitted by law to me electronically by:
  
  
  • sending it to me by email (which may include a link to a website from where it can be downloaded); and/or
    
    
• • making it available to me on a website from where it can be downloaded.
    
    
• acknowledge that where the trustee is unable to accept certain contributions made by me or on my behalf, then those contributions will be returned by the trustee to me or the person or organisation (including my employer) who made the contribution on my behalf and any such contribution will not be added to my super account balance.
  
  
• agree to receive the Product Disclosure Statement (PDS) for the fund electronically.
  
  
• acknowledge that I have read and understood the PDS for my Crescent Wealth Super account and agree to be bound by it. I understand that the PDS contains a number of references to important information set out in a number of documents and they should be read before I make a decision about opening my account with Crescent Wealth.
  
  
• understand that all of my super will be invested in my chosen investment option.
  
  
• understand that an account in the Crescent Wealth Superfund will be created using the details provided on the online application form. Online access to this account will be given to any person who uses my login details. I agree to release and indemnify the trustee and its related entities from and against all liabilities which may be suffered by me or brought against the trustee and its related entities in respect of my or my representatives’ acts or omissions in relation to the Crescent Wealth Superfund.
  
  
• agree to be bound by all the terms and conditions of the Crescent Wealth website and acknowledge that Mercer Outsourcing (Australia) Pty Ltd and its related entities may vary its terms and conditions in respect of the website or suspend access at any time.
  
  
• declare that all information set out in the online application form is true and correct on the date it is provided.
